ABSTRACT

ABSTRACT BACKGROUND AND OBJECTIVES: Health literacy is a topic that has been gaining traction in recent years, both on the policy and research agendas of many countries, however there is no evidence about temporomandibular disorders (TMD) and this topic. The objective was to evaluate the association between the level of functional literacy and the understanding of the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) through self-report and to associate the time of TMD complaint with the levels of functional literacy. METHODS: This is an observational, cross-sectional study, comprising 56 patients from the Temporomandibular Disorders (TMD) clinic at São Leopoldo Mandic College. The DC/TMD questionnaire - axis I, a sociodemographic questionnaire, and tests of the Indicador de Alfabetismo Funcional (Functional Literacy Indicator - INAF) were applied. RESULTS: The outcomes showed that 85.7% of the sample were females and 14.3% males, mainly white (60.7%), with a schooling degree, predominantly from the 3rd grade of high school and professionally active. It was found that 22.5% of the sample were functionally illiterate and 77.5% were functionally literate, 30.6% of whom had elementary level schooling. There was a significant association between the schooling level and combined INAF (p=0.005), as well as between combined INAF and duration of complaint in intervals for jaw pain (p=0.037) when Chi-square tests were used. There is evidence to establish that the mean duration of headache complaints (in months) between literacy groups are different (p=0.001) when using the Student´s t-test. With these results, it is possible to suggest that lower levels of functional literacy are related to a poorly detailed perception of TMD complaints regarding the duration of the complaint and, mainly, to the association of headache with a complaint of TMD. CONCLUSION: Lower levels of functional literacy are related to a poorly detailed perception of TMD complaints regarding the duration of the complaint and, mainly, to the association of headache with a complaint of temporomandibular disorders.


RESUMO JUSTIFICATIVA E OBJETIVOS: O letramento em saúde é um tema que vem ganhando força nos últimos anos, tanto nas agendas políticas como de investigação de muitos países, no entanto não existem evidências sobre as disfunções temporomandibulares e este tema. O objetivo foi avaliar a associação entre o nível de alfabetismo funcional e a compreensão dos Critérios Diagnósticos para Disfunção Temporomandibular (DC/DTM) por meio do autorrelato e associar o tempo de queixa de DTM com os níveis de letramento funcional. MÉTODOS: Trata-se de um estudo observacional, transversal, realizado com 56 pacientes do ambulatório de Disfunções Temporomandibulares (DTM) da Faculdade São Leopoldo Mandic. Para isso foram aplicados o questionário DC/DTM - eixo I, questionário sociodemográfico e testes do Indicador de Alfabetismo Funcional (INAF). RESULTADOS: Mostraram que 85,7% da amostra era do sexo feminino e 14,3% do sexo masculino, predominantemente brancos (60,7%), com escolaridade superior, predominantemente do 3º ano do ensino médio e profissionalmente ativos (57,1%). Verificou-se que 22,5% da amostra eram analfabetos funcionais e 77,5% eram alfabetizados funcionalmente, sendo que 30,6% possuíam nível fundamental. Houve associação significativa entre escolaridade e INAF combinado (p=0,005), bem como entre INAF combinado e tempo de queixa nos intervalos de dor mandibular (p=0,037) quando foram utilizados testes Qui-quadrado. Há evidências que estabelecem que a duração média das queixas de cefaleia (em meses) entre os grupos de alfabetismo é diferente (p=0,001) quando utilizado o teste t de Student. Com base nesses resultados, é possível sugerir que níveis mais baixos de alfabetismo funcional estariam relacionados a uma percepção pouco detalhada das queixas de DTM quanto à duração da queixa e à associação de cefaleia com queixa de DTM. CONCLUSÃO: Níveis mais baixos de alfabetismo funcional foram relacionados à percepção pouco detalhada das queixas de DTM quanto à duração e, principalmente, à associação de cefaleia com queixa de disfunção temporomandibular.

Introducción: la artritis reumatoide es parte del grupo de las enfermedades autoinmunes con incidencia considerable sobre la población. Se caracteriza por la afección de las articulaciones del cuerpo que la padece; en mayor frecuencia se encuentra afectada la articulación temporomandibular por el complejo articular que ésta presenta; entre los signos y síntomas que comúnmente podemos encontrar en pacientes con este tipo de enfermedad son los chasquidos o ruidos articulares, dolor orofacial, pérdida o imposibilidad del movimiento de la mandíbula y cambios anatómicos localizados en el área de la articulación temporomandibular. Objetivo: describir las consecuencias que desencadena la artritis reumatoide sobre la articulación temporomandibular y cómo es para el odontólogo el manejo de estos pacientes en consulta, evaluar los tratamientos para cada caso sobre un correcto diagnóstico. Material y métodos: se realizó una revisión bibliográfica de artículos recientes sobre el tema, utilizando buscadores como SciELO, Elsevier y PubMed, siendo 30 las fuentes seleccionadas con idiomas en inglés y español. Resultados: esta enfermedad autoinmune se caracteriza por afectar múltiples articulaciones del cuerpo humano simétrica y bilateralmente incluyendo la articulación temporomandibular (ATM), lo cual conlleva al riesgo de desarrollar trastornos temporomandibulares (TTM). Es importante conocer los métodos para realizar un correcto diagnóstico oportuno de la ATM del paciente con artritis reumatoide (AR) con la finalidad de ofrecer un tratamiento conservador. Conclusión: los trastornos temporomandibulares desencadenantes de la artritis reumatoide son afecciones que se deben considerar para el buen manejo del paciente con este padecimiento, comprender y respaldar un diagnóstico clínico es de vital importancia para dar al paciente un tratamiento adecuado dependiendo el grado de complejidad en la que cada individuo se encuentra; conocer el manejo adecuado y encaminar al paciente a una mejor calidad de vida es clave en la consulta odontológica del día a día (AU)


Introduction: rheumatoid arthritis is part of the group of autoimmune diseases with considerable incidence in the population. It is characterized by the affection of the joints of the body that suffers from it; most frequently the temporomandibular joint is affected due to the articular complex that it presents; among the signs and symptoms that we can commonly find in patients with this type of disease are joint clicks or noises, orofacial pain, loss or impossibility of jaw movement and anatomical changes located in the temporomandibular joint area. Objective: to describe the consequences that rheumatoid arthritis triggers on the temporomandibular joint and how it is for the dentist to manage these patients in consultation, to evaluate the treatments for each case on a correct diagnosis. Material and methods: a bibliographic review of recent articles on the subject was carried out, using search engines such as SciELO, Elsevier and PubMed, with 30 sources selected in English and Spanish. Results: this autoimmune disease is characterized by affecting multiple joints of the human body symmetrical and bilaterally including the TMJ which leads to the risk of developing TMD. It is important to know the methods to make a correct diagnosis of the TMJ of the patient with RA in order to offer a conservative treatment. Conclusions: the temporomandibular disorders that trigger rheumatoid arthritis are conditions that should be considered for the proper management of the patient with this condition, understanding and supporting a clinical diagnosis is of vital importance to give the patient an adequate treatment depending on the degree of complexity in which each individual is; knowing the proper management and directing the patient to a better quality of life is key in the day-to-day dental practice (AU)

Objetivo: Esta pesquisa teve como objetivo comparar as manifestações clínicas da disfunção temporomandibular (DTM) em indivíduos com dentição natural e usuários de prótese total dupla. Métodos: A amostra do estudo consistiu em 120 participantes, distribuídos em dois grupos de 60 indivíduos cada. O Grupo 1 era composto por pessoas com dentição natural, enquanto o Grupo 2 era composto por indivíduos que utilizavam próteses totais duplas. Para identificar os sinais e sintomas da DTM, foram realizados tanto o índice anamnésico quanto o exame físico específico. Resultados: Os indivíduos que utilizavam prótese total apresentaram um aumento de 2,63 vezes no risco de apresentar dor de ouvido e um aumento de 5,42 vezes de ter limitação do movimento lateral da mandíbula. Por outro lado, os indivíduos com dentição natural apresentaram maior sensibilidade à palpação dos músculos temporal anterior e temporal posterior, além de um risco de 0,27 vezes maior de ter sensibilidade articular. Conclusões: Concluiu-se que os principais sinais clínicos de DTM dos indivíduos dentados naturais foram a sensibilidade à palpação muscular e a sensibilidade à palpação articular e no grupo de desdentados a limitação de movimento lateral da mandíbula e o sintoma de dor de ouvido foram mais frequentes.


Objective: This research aimed to compare the clinical manifestations of temporomandibular dysfunction (TMD) in individuals with natural dentition and users of double total prosthesis. Methods: The study sample consisted of 120 participants, distributed in two groups of 60 individuals each. Group 1 was composed of people with natural dentition, while Group 2 was composed of individuals who used double total prostheses. To identify the signs and symptoms of TMD, both the anamnestic index and the specific physical examination were performed. Results: The individuals who used total prosthesis showed an increase of 2.63 times in the risk of presenting ear pain and an increase of 5.42 times of having limitation of the lateral movement of the jaw. On the other hand, individuals with natural dentition showed greater sensitivity to palpation of the anterior and posterior temporal muscles, in addition to a 0.27 times greater risk of joint tenderness. Conclusions: It was concluded that the main clinical signs of TMD of natural dentate individuals were the sensitivity to muscle palpation and the sensitivity to joint palpation and in the group of toothless individuals the limitation of lateral jaw movement and the ear pain symptom were more frequent.


Propósito: El objetivo de esta investigación fue comparar las manifestaciones clínicas de disfunción temporal (DTM) en individuos con dientes naturales y usuarios de prótesis doble total. Métodos: La muestra de estudio consistió en 120 participantes, distribuidos en dos grupos de 60 individuos cada uno. El grupo 1 estaba formado por personas con dientes naturales, mientras que el grupo 2 estaba formado por individuos con prótesis dobles totales. Para identificar los signos y síntomas de la DTM, se realizó tanto el índice anamésico como el examen físico específico. Resultados: los individuos que utilizaron la prótesis total mostraron un incremento de 2,63 veces en el riesgo de sufrir un dolor de oído y un aumento de 5,42 veces en la limitación del movimiento lateral de la mandíbula. Por otro lado, los individuos con dientes naturales mostraron mayor sensibilidad a la palpación de los músculos temporales anteriores y posteriores, además de un riesgo de 0,27 veces mayor de sensibilidad articular. Conclusiones: Se concluyó que los principales signos clínicos de MSD en sujetos con dientes naturales fueron sensibilidad a la palpación muscular y sensibilidad a la palpación articular y al grupo sin dientes, limitación del movimiento lateral de la mandíbula y síntoma de cefalea fueron más frecuentes.

ABSTRACT

El objetivo de este estudio de revisión sistemática consistió en la búsqueda de hallazgos clínicos en estudios de calidad sobre la efectividad de la terapia con férulas de descarga oclusales. Esta investigación bibliográfica se realizó en PubMed, Embase, SciELO, Science Direct, Scopus, Google Scholar y en el Registro Central de Ensayos clínicos Cochrane; delimitando la búsqueda desde el 1 de enero de 2011 al 1 de junio de 2022. Se incluyeron un total de 21 artículos, todos relacionados con la disfunción de la articulación temporomandibular y la eficacia de las férulas de descarga oclusales como tratamiento. Las férulas de descarga oclusales reducen eficazmente los síntomas dolorosos en pacientes con trastornos temporomandibulares, tanto en patologías musculares como articulares, aunque con mayor eficacia en casos de disfunción muscular, ya que se han observado desplazamientos recurrentes de disco en patologías articulares.


The objective of this systematic review study was to search for clinical findings in quality studies on the effectiveness of occlusal splint therapy. This literature search was conducted in PubMed, Embase, SciELO, Science Direct, Scopus, Google Scholar, and the Cochrane Central Register of Clinical Trials; delimiting the search from January 1, 2011 to June 1, 2022. A total of 21 articles were included, all related to temporomandibular joint dysfunction and the efficacy of occlusal splints as a treatment. Occlusal splints effectively reduce painful symptoms in patients with temporomandibular disorders, both in muscular and joint pathologies, although more effectively in cases of muscular dysfunction, since recurrent disc displacements have been observed in joint pathologies.

ABSTRACT

O Bruxismo é um comportamento da musculatura mandibular, caracterizado pelo apertamento e/ou rangimento dental, tanto em estado de vigília quanto no sono, que pode acarretar na Disfunção Temporomandibular (DTM). Altamente prevalente em crianças e etiologia multifatorial, destacam-se os aspectos psicossociais, principalmente o estresse e ansiedade. A necessidade de distanciamento social durante a pandemia da COVID-19 trouxe consequências para as crianças e fez surgir novos formatos de ações na Odontologia. Esse estudo avaliou a influência do autocuidado nas manifestações do Bruxismo/DTM em crianças que estavam em tratamento, antes da pandemia, no Serviço de Atendimento de Bruxismo e DTM na Infância (SABDI) da FORP/USP, por meio de intervenções assistidas, implementadas remotamente, de Práticas Integrativas e Complementares (PICs). Previamente às intervenções (T0), os pais responderam o questionário (Q1) sobre a criança (no formato Google Forms, enviado por WhatsApp e e-mail). As práticas propostas foram explicadas em reuniões virtuais (Google Meet) e foram realizadas durante 21 dias, registradas em um "Diário de Controle". Um novo questionário (Q2) foi preenchido para análise comparativa após as intervenções (T1). Das 37 crianças que estavam em tratamento, a amostra final das 18 que concluíram todas as etapas propostas apresentou diferença quantitativa nos itens: "dor na boca/rosto quando mastiga"; "dor ou dificuldade de abrir e fechar a boca"; "acorda com dor no rosto ou dor de cabeça" e, "dor de cabeça durante o dia ou à noite", "range os dentes em vigília"; "frequência que range os dentes em vigília"; "range os dentes enquanto dorme" e, "frequência que range os dentes enquanto dorme", "sono agitado"; "pesadelo" e "insônia". Observou-se evidência de diferença estatística com valor P<0,05, no item "aperta os dentes em vigília" (p=0,0057). Conclui-se que as intervenções propostas influenciaram as manifestações do Bruxismo favorecendo o seu controle.


Bruxism is a behavior of the mandibular muscles, characterized by clenching and/or dental grinding, both in wakefulness and in sleep, which can lead to Temporomandibular Disorders (TMD). Highly prevalent in children and possessing a multifactorial etiology, psychosocial aspects stand out, especially stress and anxiety. The need for social distancing during the COVID-19 pandemic has had consequences for children and has given rise to new formats of actions in Dentistry. This study evaluated the influence of self-care on the manifestations of Bruxism/TMD in children who were undergoing treatment, before the pandemic, Care Center for children with Bruxism and TMD, whose acronyms in Portuguese is SABDI, located in Ribeirão Preto School of Dentistry (FORP-USP), through assisted interventions, implemented remotely, of Complementary and Integrative Therapies (CIT). Prior to the interventions (T0), the parents answered the questionnaire (Q1) about the child (in Google Forms, sent by WhatsApp and e-mail). The proposed practices were explained in virtual meetings (Google Meet) and performed for 21 days and recorded in a "Control Diary". A new questionnaire (Q2) was completed for comparative analysis after the interventions (T1). Of the 37 children who were undergoing treatment, the final sample of 18 who completed all the proposed steps showed a quantitative difference in the items: "pain in the mouth/face when chewing"; "pain or difficulty opening and closing the mouth"; "waking up with pain in the face or headache" and "headache during the day or night"; "grinding teeth while awake"; "frequency grinding teeth while awake"; "grinding teeth while sleeping"; "frequency grinding teeth while sleeping"; "restless sleep"; "nightmare"; and "insomnia". Evidence of statistical difference was observed with a P value <0.05, in the item "clenches teeth while awake" (p=0.0057). It is concluded that the proposed interventions influenced the manifestations of Bruxism, favoring its control.

ABSTRACT

Introdução: A articulação temporomandibular (ATM) é uma estrutura especializada e excepcional, relacionada com funções estomatognáticas e propensa à disfunções associadas ao sistema maxilo-mandibular. Dentre essas alterações, a luxação é uma entidade patológica importante. Possui patogênese multifatorial e pode se manifestar de modo recidivante. Diversos tratamentos são propostos, dentre eles, a criação de anteparos aloplásticos. Paciente do sexo feminino, 22 anos com história de luxação mandibular recidivante com sintomatologia congruente ao quadro. Realizou-se tratamento cirúrgico com instalação de miniplacas de titânio do sistema 2.0 mm em forma de duplo T com curvatura de aproximadamente 90°, na região de eminencia articular. Após 1 ano e 6 meses, a paciente segue assintomática, sem lesões nervosas e sem luxações. A luxação da ATM é um quadro clínico angustiante, sendo a segunda luxação mais frequente e incidente em mulheres. Caracteriza-se por: incapacidade de fechar a boca, depressão cutânea pré-auricular, ptialismo e musculatura mastigatória tensa. A paciente apresentava severas luxações recidivantes, juntamente com ansiedade e angústia social. A utilização de placas evitou a hiperexcussão e remissão do quadro. É um procedimento menos agressivo e reversível. A técnica utilizada demonstrou bom prognóstico e, dentre as opções cirúrgicas, é uma técnica mais conservadora... (AU)


Introduction: TMJ dislocation is a multifactorial joint pathology that occurs when the mandibular condyle exceeds the articular eminence and cannot return to its original anatomical position, making it impossible for the patient to close the mouth. A 22-year-old female patient complaining of succes sive episodes of mandibular dislocation, characterizing the condition of dislocation redicivant TMJ. Surgical treatment was performed with the installation of 2.0 mm double T-shaped titanium miniplates with approximately 90° curvature in the region of articular eminence. After 8 months, the patient has no complaints, no signs of facial nerve damage and no episodes of dislocation. TMJ dislocation is a socially and psychologically distressing clinical condition, being the second most frequent dislocation in the body and with higher incidence in female patients. Key features are inability to close the mouth, pre auricular cutaneous depression, excessive salivation, and tense and spasmodic masticatory muscles. In this case, the patient presented severe relapsing dislocations, along with anxiety and social anguish. The use of plates acts as a mechanical barrier for condylar movements, avoiding hyperexcussion, being a less aggressive and reversible procedure, having only the disadvantage of the possibility of plate frac ture. RESUMEN El objetivo de la investigación fue determinar el diagnóstico y la prevalencia de la Disfunción Temporomandibular (DTM), en el Hospital General María Auxiliadora, Lima, 2019. Material y métodos. Trabajo descriptivo, prospectivo, longitudinal y al azar. La muestra se constituyó con 76 pacientes, aplicándose los Criterios de Investigación para Diagnóstico de Desórdenes Témporo-Mandibulares(RDC/TDM). Resultados. Encontramos, que 73.7% fue del sexo femenino, especialmente en el grupo etario de 30-39 años (35.6%). La etiología de la DTM, es multifactorial y está relacionada a factores estructurales, funcionales y psicológicos. En el diagnóstico de DTM, la cefalea (38.1%) fue el principal síntoma y el dolor a la palpación de la articulación témporomandibular (51.4%), fue el signo destacado en el examen clínico, especialmente, en el sexo femenino (38.2%). El trastorno mixto de Ansiedad-Depresión, fue el cuadro clínico, de alteración del estado de salud mental más frecuente (50.0%), asociado a DTM, predominando en el sexo femenino (38.2%). La combinación de exámenes clínico-otorrinolaringológico, permitió el diagnóstico de DTM, en 77.6% de pacientes. Conclusión. Los pacientes con cuadros clínicos de DTM, que cursan con cefalea y concomitantemente con Ansiedad-Depresión, requieren un manejo terapéutico integral y precoz, para restituirlos a su ambiente de trabajo y mejorar su productividad laboral; la DTM, debería ser considerada, como una causa frecuente de cefalea.


ABSTRACT The objective of this study was to determine the diagnosis and prevalence of temporomandibular joint dysfunction (TMJD) in Maria Auxiliadora General Hospital in Lima, Peru, during 2019. Material and Methods: This is a prospective randomized longitudinal descriptive study. The sample included 76 patients, and Research Criteria for Diagnosing Temporomandibular Joint Disorders were used. Results: We found that 73.7% patients were female, particularly belonging to the 30-39 years age group (35.6%). TMJD etiology is multifactorial and it is related to structural, functional, and psychological factors. When diagnosing TMJD, headache was the main symptom (38.1%), and pain on palpation (51.4%) was the most prominent sign on clinical examination, particularly in females (38.2%). Mixed anxiety-depressive disorder, was the most frequent mental status alteration associated with TMJD found on clinical examination (50.0%), and it also was more frequently found in females (38.2%). A combination of general clinical and ear, nose, and throat examination allowed reaching a diagnosis in 77.6% of all subjects. Conclusion: Patients with TMJD who present with headache and concomitantly with anxiety-depression, require integrative and early therapy management, so they may be reinstalled to their working environment and improve their productivity. TMJD should be considered as a frequent cause of headache.

ABSTRACT

Resumo Introdução Ocorrendo de forma branda e não invasiva, a terapia craniossacral é uma técnica de manipulação na qual o terapeuta exerce leve pressão sobre estrutura óssea, sendo utilizada como tratamento para diversos problemas de saúde, como cefaleias e DTM. Objetivo Analisar os efeitos da terapia craniossacral em indivíduos com disfunção temporomandibular associada a cefaleia do tipo tensional. Material e método Estudo descritivo, intervencionista com abordagem quantitativa, realizado no núcleo de atenção médica integrado, no período de fevereiro a setembro de 2018. Foram inclusos no estudo indivíduos com disfunção temporomandibular e que apresentaram cefaleia dentre os sintomas. Foram exclusos aqueles que não compareceram ao atendimento ou não apresentaram a sintomatologia. Previamente, foi realizada uma avaliação para identificar a dor na crise de cefaleia, a mensuração da amplitude de movimento mandibular e a palpação dos músculos da mastigação, para classificar a dor em leve, moderada ou forte. Após oito atendimentos, sendo duas vezes por semana com duração de 15 minutos cada, todos foram reavaliados. Resultado Participaram do estudo 31 indivíduos, sendo 90,3% do sexo feminino. Na classificação do RDC/TMD, houve prevalência dos grupos G1 e G1G3. Quanto a mobilidade mandibular, houve ganho para os movimentos de abertura, com 45,6 mm (±7,5) antes da terapia e, na reavaliação, 47,4 mm (±8,4); desvios laterais antes da terapia (lado direito - 7,0 ±2,8) e (lado esquerdo - 7,7±3,0), e após a terapia (8,0±3,0) e (8,6±2,9), respectivamente; o movimento de protusão, antes com média 5,03 ±2,5 e, na reavaliação, com 4,8 ±1,9. Na palpação muscular, destacamos redução do quadro álgico no músculo pterigoideo medial com média 2,2 (±1,05) antes da terapia e 1,5 (±1,02) após a terapia. Conclusão Concluímos que a terapia se mostrou eficaz no tratamento de pacientes com disfunção temporomandibular associada a cefaleia do tipo tensional.


Abstract Introduction Occurring in a mild and non-invasive way, craniosacra therapy is a manipulation technique that the therapist exerts light pressure on the bone structure, being used as a treatment for several health problems, such as headaches and TMD. Objective To analyze the effects of craniosacra therapy in individuals with temporomandibular disorders associated with tension-type headache. Material and method Descriptive, interventional study with a quantitative approach, carried out in the integrated medical care center from February to September 2018. Individuals with temporomandibular disorder and who presented headache among the symptoms were included in the study. And those who did not attend the service or did not present symptoms were excluded. Previously, an assessment was carried out to identify pain during headache attacks, measurement of mandibular range of motion and palpation of mastication muscles, to quantify pain as mild, moderate and severe. After 8 consultations, twice a week lasting 15 minutes each, all were reassessed. Result 31 individuals participated in the study, being 90.3% female. In the classification of the RDC/TMD, there was a prevalence of groups G1 and G1G3. Regarding mandibular mobility, there was a gain for opening movements with 45.6mm (±7.5) before therapy and 47.4mm (±8.4) in the reassessment; lateral deviations before therapy (right side - 7.0 ±2.8) and (left side - 7.7±3.0) and after (8.0±3.0) and (8.6±2.9); and the protrusion movement before with an average of 5.03 ± 2.5 and in the reassessment with 4.8 ± 1.9. And in muscle palpation, we highlight a reduction in pain in the medial pterygoid muscle with a mean of 2.2 (±1.05) before therapy and 1.5 (±1.02) after. Conclusion We conclude that the therapy has been shown to be effective in treating patients with temporomandibular disorders associated with tension-type headache.

ABSTRACT

Objetivo: evaluar la rehabilitación funcional de la ATM en pacientes con osteoartrosis, con abordaje neuromuscular. La osteoartrosis se caracteriza por la degeneración del cartílago articular y la cortical ósea, que conduce a dolor e inmovilidad. Se incluyó a 8 mujeres entre 20 y 42 años de edad con dolor en la región orofacial, signos de oclusión disfuncional e imágenes compatibles con trastornos degenerativos en ATM. Clínicamente se evaluó el dolor y la oclusión y, mediante tecnología electrónica, se obtuvieron datos objetivos de los movimientos de apertura-cierre y lateralidades, la velocidad de apertura-cierre mandibulares y de los ruidos articulares. Las mediciones fueron realizadas al inicio y al finalizar el tratamiento de estabilización mandibular con una ortosis neuromuscular. Para determinar la significación estadística se utilizaron el Student`s test de comparaciones múltiples y el análisis de varianza, ANOVA, de un factor. Las diferencias entre medias se consideraron significativas con p <0.05. Los resultados demostraron ausencia de dolor en el 100% de los casos al primer mes de tratamiento. Se incrementaron la apertura bucal (6,73 mm promedio), ambas lateralidades (P=0.0023), velocidad en apertura y en cierre (no estadísticamente significativo). Después de estabilizar la mandíbula se redujo la frecuencia de los ruidos. En conclusión, la oclusión dental disfuncional es el principal factor etiológico de la artrosis de la ATM. El establecimiento de una oclusión fisiológica produce la descompresión de la ATM, aumenta el rango de los movimientos mandibulares y disminuye significativamente el dolor (AU)


Objective: to evaluate the functional rehabilitation of the TMJ in patients with osteoarthritis, with a neuromuscular approach. Osteoarthrosis is characterized by degeneration of articular cartilage and bone cortex, which leads to pain and immobility. Eight women between 20 and 42 years of age with pain in the orofacial region, signs of dysfunctional occlusion, and images consistent with degenerative TMJ disorders were included. Clinically, pain and occlusion were evaluated and, using electronic technology, objective data were obtained on the opening-closing movements and lateralities, the opening-closing speed of the jaws and joint noises. Measurements were made before and at the end of the mandibular stabilization treatment with a neuromuscular orthosis. To determine the statistical significance, the Student`s multiple comparisons test and the analysis of variance, ANOVA, of one factor were used. The differences between means were considered significant with p <0.05. The results showed absence of pain pain in 100% of cases in the first month of treatment. Mouth opening (6.73 mm average), both lateralities (P = 0.0023), opening and closing speed (not statistically significant) were increased. After stabilizing the jaw the noise frequency value was reduced. In conclusion, dysfunctional dental occlusion is the main etiologic factor of TMJ osteoarthritis. Establishing a physiological occlusion causes TMJ decompression, increases the range of mandibular movements, and significantly decreases pain (AU)

ABSTRACT

Objetivo: realizar uma revisão da literatura a fim de traçar um panorama atual sobre o papel do estrogênio nas disfunções temporomandibulares. Metodologia: foram realizadas buscas nas plataformas digitais Pub- Med, SciELO e Google Acadêmico, durante os meses de setembro de 2018 a maio de 2019, sem filtros para determinar período de tempo, sendo excluídos aqueles trabalhos em que não foi possível identificar relação com o tema. Resultados: na análise dos estudos encontrados, foi observada a relação entre o estrogênio e a prevalência de disfunções temporomandibulares em mulheres. O estrogênio atua central e perifericamente no sistema nervoso central, influenciando no processamento dos receptores pró e antinoceptivos da articulação temporomandibular. Considerações finais: a modulação estrogênica da dor é um mecanismo complexo. Diversos estudos associam o hormônio estrogênio às disfunções temporomandibulares. Embora não haja um consenso entre os autores do papel exato deste hormônio, há evidências comprovadas de que as mulheres possuem uma susceptibilidade a dores em geral, com prevalência tanto em frequência quanto em intensidade.(AU)


Objective: to carry out an integrative review of the literature in order to outline the role of estrogen in temporomandibular disorders. Methodology: the study searched studies in the digital platforms PubMed, SciELO and Google Acadêmico, from September 2018 to May 2019, without filters to determine the time period, excluding those works in which it was not possible to identify relation with the theme. Results: among the analysis of the studies found, the relationship between estrogen and the prevalence of temporomandibular disorders in women was observed. Estrogen acts centrally and peripherally in the central nervous system, influencing the processing of the pro and antinoceptive receptors of the temporomandibular joint. Final considerations: estrogenic modulation of pain is a complex mechanism. Several studies associate the estrogen hormone with temporomandibular disorders. Although there is no consensus among authors of the exact role of this hormone, there is proven evidence that women have a susceptibility to pain in general, with prevalence both in frequency and intensity.(AU)

Resumen Introducción: La disfunción temporomandibular (DTM) puede estar ocasionada por numerosos factores, como maloclusiones, traumatismos, bruxismo, así como por estrés y depresión. Esta última se caracteriza por un estado de profunda tristeza, abatimiento e infelicidad. Ambos trastornos pueden presentarse en la adolescencia, cuando se crean las pautas de lo que será la persona en la adultez, tanto en el aspecto psicológico como en el emocional y el anatomofisiológico. El objetivo de este estudio fue determinar la relación del grado de DTM con el grado de depresión en adolescentes de ascendencia maya de la comunidad de Yobaín, Yucatán, México. Métodos: Se llevó a cabo un estudio de tipo correlacional y transversal. Se incluyeron 210 adolescentes de ascendencia maya de entre 10 y 19 años de edad. Para el estudio se utilizaron el índice de Helkimo y la escala de depresión de Beck. Se utilizó estadística descriptiva e inferencial. Resultados: El 74.3% de los adolescentes presentó algún grado de DTM y el 51.9%, algún grado de depresión. Se encontró que a mayor grado de DTM, mayor es el grado de depresión. La frecuencia de DTM es similar tanto para la adolescencia temprana como para la tardía, aunque en la temprana se observó mayor frecuencia de depresión grave. Conclusiones: A falta de otros estudios similares, se logró sentar un precedente con este estudio acerca el estado de salud psicológica y odontológica en esta población de origen yucateco. Se estableció una pauta para acudir a otras áreas de atención médica, como psiquiatría, neurología y neuropsiquiatría.


Abstract Introduction: Temporomandibular dysfunction (TMD) is caused by multiple factors such as depression, which is characterized by a state of deep sadness, dejection and unhappiness that can be transitory or permanent. Both of these disorders can occur in adolescence, when guidelines are created for what the person will be like in adulthood for the psychological, emotional and anatomophysiological aspects. The objective of this study was to determine the relationship between the degrees of DTM and depression in adolescents of Mayan ancestry from the community of Yobain, Yucatan, Mexico. Methods: A correlational and transversal study was conducted, using the Helkimo index and the Beck depression scale, in which 210 adolescents aged 10 to 19 years of Mayan descent were included. Descriptive and inferential statistics were used. Results: Some degree of DTM was observed in 74.3% of the participants, and some degree of depression in 51.9%. A higher degree of DTM was associated with a greater degree of depression. The frequency of DTM was similar for both categories of adolescence: early and late. However, a higher frequency of severe depression was observed in early adolescence. Conclusions: In the absence of other similar studies, a precedent was set with this study on the state of psychological and dental health in this population of Yucatan origin. Also, a guideline was established for referral to other areas of health care, such as psychiatry, neurology and neuropsychiatry.

Introducción: El síndrome de disfunción temporomandibular constituye un problema de salud importante, que afecta a más de 50 % de la población mundial. Objetivo: Describir la evolución terapéutica de pacientes con afecciones temporomandibulares tratados con farmacopuntura. Métodos: Se realizó un estudio descriptivo de 60 pacientes con síndrome de disfunción temporomandibular en fase aguda, atendidos en el Hospital Clinicoquirúrgico Faustino Pérez de Matanzas, desde enero hasta diciembre de 2018. Se administró una solución compuesta por lidocaína, vitamina B12 y betametasona en puntos acupunturales determinados. Además, se utilizó la escala verbal del dolor. Resultados: Se encontró que el grupo etario de 60 años y más, así como el sexo femenino fueron los más afectados. De los factores asociados a este síndrome predominó el estrés en 100 % de los pacientes; 63,3 % presentaron dolor moderado y 53,3 % se aliviaron totalmente. Conclusiones: La farmacopuntura alivió el dolor asociado al síndrome de disfunción temporomandibular en la mayoría de los afectados, entre las 3 primeras sesiones de tratamiento.


Introduction: The temporomandibular dysfunction syndrome constitutes an important health problem that affects more than 50 % of the world population. Objective: To describe the therapeutic clinical course of patients with temporomandibular affections treated with pharmacopuncture. Methods: A descriptive study of 60 patients with temporomandibular dysfunction syndrome in acute phase was carried out, they were assisted in the Faustino Pérez Clinical Surgical Hospital from Matanzas, from January to December, 2018. A solution composed by lidocaine, B12 vitamin and betamethasone was administered in certain acupunctural points. Also, the pain verbal scale was used. Results: It was found that the 60 years and over age group, as well as the female sex were those most affected. Of the factors associated to this syndrome stress prevailed in 100 % of patients; 63.3 % presented moderate pain and 53.3 % were totally relieved. Conclusions: Pharmacopuncture relieved the pain associated with the temporomandibular dysfunction syndrome in most of those affected, in the first 3 treatment sessions.

ABSTRACT BACKGROUND AND OBJECTIVES: Temporomandibular dysfunction consists of frequent non-dental pain in the orofacial region of multifactorial origin and interdisciplinary treatment, among them, acupuncture. The treatment of temporomandibular dysfunction acts both in muscle relaxation and pain control, trying to achieve the physical, mental, and emotional balance of the patient, thus reducing anxiety and improving the quality of life. The objective of this study was to evaluate acupuncture as a treatment for temporomandibular dysfunction. METHODS: A total of 34 volunteers screened and selected at the Federal University of Mato Grosso do Sul, diagnosed with muscle dysfunction according to the Research Diagnostic Criteria. They were randomly divided into two equal groups: group 1 treated with occlusal plaque, massage, thermotherapy and self-care guidelines; and group 2 treated with six acupuncture sessions lasting 30 minutes each. The pain was evaluated by the visual analog scale, and an algometer to assess the muscular tension of the temporal and masseter muscles. The limitation of mouth opening was measured with the use of calipers. The Mann-Whitney test was used for the non-normal distribution (visual analog scale and tension threshold) between the two groups (G1 and G2), and the Friedman test to compare the assessment periods (beginning of the treatment, after six weeks and four months) with a significance level of 5%. RESULTS: There was no difference in mouth opening, visual analog scale scores, or muscle tension threshold in relation to the type of treatment used. Both groups improved after six weeks of treatment. There was no statistical difference in the values obtained after six weeks and after four months. CONCLUSION: The statistical results showed that acupuncture increased the muscle tension threshold, improved the mouth opening and reduced pain, being as effective as the most commonly used conventional therapies.


RESUMO JUSTIFICATIVA E OBJETIVOS: A disfunção temporomandibular consiste em dores não dentárias frequentes na região orofacial, de origem multifatorial e de tratamento interdisciplinar. Entre esses tratamentos está a acupuntura. O tratamento da disfunção temporomandibular atua tanto no relaxamento muscular quanto no controle da dor, sistemicamente buscando o equilíbrio físico, mental e emocional do paciente, diminuindo a ansiedade e melhorando a qualidade de vida. Este trabalho teve como objetivo avaliar a acupuntura como tratamento da disfunção temporomandibular. MÉTODOS: Participaram do estudo 34 voluntários triados e selecionados na universidade de Mato Grosso do Sul, diagnosticados com disfunção muscular pelo Research Diagnostic Criteria, e foram divididos aleatoriamente em dois grupos iguais. O grupo 1 foi tratado com placa oclusal, massagem, termoterapia e orientações de autocuidado. O grupo 2 foi tratado com 6 sessões de acupuntura com duração de 30 minutos cada. A dor foi avaliada pela escala analógica visual e com auxílio de um algômetro para avaliar a tensão muscular dos músculos temporal e masseter. A limitação de abertura bucal foi medida com o uso do paquímetro. Para a distribuição não normal (escala analógica visual e limiar de tensão), entre os dois grupos (G1 e G2), foi utilizado o teste Mann-Whitney, e para a comparação entre os períodos de avaliação (início do tratamento, após seis semanas e 4 meses), foi utilizado o teste de Friedman, ao nível de significância de 5%. RESULTADOS: Não houve diferença das medidas de abertura de boca, dos escores de escala analógica visual e de limiar de tensão muscular segundo o tipo de tratamento utilizado. No entanto, em ambos os grupos houve melhora nos parâmetros avaliados após seis semanas de tratamento. Não houve diferença estatística dos valores obtidos após seis semanas e ao final do tratamento, após quatro meses. CONCLUSÃO: Os resultados estatísticos mostraram que a acupuntura aumentou o limiar de tensão muscular, melhorou a abertura de boca e diminuiu a dor. Concluiu-se que a acupuntura foi tão eficiente quanto as terapias convencionais mais utilizadas.

RESUMEN: La ansiedad puede ser un detonante de la disfunción temporomandibular (DTM). Se ha mencionado que el trastorno psicológico más frecuente en México es la "ansiedad" con 14,3 %. El objetivo es correlacionar y comparar los niveles de ansiedad y los grados de DTM en cirujanos dentistas residentes (CDR) de dos universidades mexicanas: la Facultad de Odontología de la Universidad de la Salle Bajío León, Guanajuato (FOULSB), y de la Facultad de Odontología de la Universidad Autónoma (FOUADY) de Yucatán México durante el periodo de septiembre 2017 a febrero 2018. El tipo de estudio fue correlacional, comparativo de cohorte transversal, siendo las variables: ansiedad (niveles según cuestionario de autoevaluación de ansiedad estado/rasgo STAI); signos y síntomas clínicos para el diagnóstico de DTM, (CDI/TTM Grupo I Desórdenes Musculares; Grupo II Desplazamientos discales; Grupo III Artralgia, osteoartritis). El tamaño de la muestra y criterios de inclusión en ambas universidades fueron: CDR en Odontología; que aceptaron participar en el estudio, mediante la firma del consentimiento informado y voluntario; hombres y mujeres; entre 25 a 34 años de edad. Criterios de exclusión: con tratamientos ortodónticos, quirúrgicos, enfermedades sistémicas o neurológicas y que sólo el día que se tomó la muestra presentaron otalgia o cefalea. El tipo de muestreo, por conveniencia. Se utilizó estadística descriptiva e inferencial. Los resultados: 40 CDR de la FOULSB, donde el 92,5 % (n=37) manifestaron tener niveles de ansiedad bajos, y el 7,5 % (n=3) moderado. El 65,8 % (n=26) presentó diagnóstico de DTM. De los 40 CDR de la FOUADY, 97,5 % (n=39) declararon tener niveles de ansiedad bajos, y el 2,5 % (n=1) obtuvo un nivel alto. El 67,5 % (n=27) presentaron signos y/o síntomas de DTM. La conclusión fue que no existieron diferencias estadísticamente significativas, ya que los CDR de ambas universidades presentaron porcentajes de DTM similares y también tuvieron niveles de ansiedad similares.


ABSTRACT: Anxiety can be a trigger for temporomandibular dysfunction (TMD). It has been mentioned that the most frequent psychological disorder in Mexico is "anxiety" with 14.3 %. Exclusion criteria: with orthodontic, surgical treatments, systemic or neurological diseases and that only the day the sample was taken showed otalgia or headache. The type of sampling, for convenience. Descriptive and inferential statistics were used. The results: 40 CDR of FOULSB, where 92.5 % (n = 37) reported having low anxiety levels, and 7.5 % (n = 3) moderate. Sixty-five point eight percent (n=26) presented diagnosis of TMD. Of the 40 CDRs of FOUADY, 97.5 % (n = 39) reported having low anxiety levels, and 2.5 % (n = 1) had a high level. Sixtyseven point five percent (n=27) presented signs and / or symptoms of TMD. The conclusion was that there were no statistically significant differences, since the CDRs of both universities presented similar DTM percentages and had similar levels of anxiety.

Introdução:A Disfunção Temporomandibular (DTM) é uma condição bastante frequente na população mundial e a identificação de fatores causais, junto ao seu tratamento é de suma relevância para a qualidade de vida dos indivíduos.Objetivo:Objetivou-se por meio de uma revisão integrativada literatura de todos os tipos de estudos, avaliar se a alteraçãode dimensão vertical impacta no aparecimento das disfunções temporomandibulares.Método:As estratégias de busca foram realizadas nas bases de dados "Cochrane Library", "MEDLINE", "Web of Science", "Scopus", "LILACS", "Scielo" e "Google Acadêmico", utilizando os seguintes descritores e/ou palavras: "Temporomandibular Joint Disorders"; "Craniomandibular Disorders"; "Occlusion Vertical Dimension"; "Occlusion Vertical Dimensions"; "Vertical Dimension of Occlusion"; "Vertical Dimension".Resultados:Um total de 4 artigos foram incluídos nesta revisão.A maioria dos estudos demostram que na presença de uma DVO diminuída, o sistema estomatognático é capaz de adaptar-se, não provocando o aparecimento de DTM.Conclusões:Como conclusão, os resultados indicam que não há evidência científica suficiente que permita afirmar que a perda de dimensão vertical predispõe ao aparecimento de sinais e sintomas relacionados à disfunção temporomandibular (AU).


Introduction:TemporomandibularDysfunction (TMD) is a very frequent condition in the world population and the identification of causal factors, along with its treatment, is extremely relevant to the quality of life of the individuals.Objective:The aim of this study was to integrativereview the literature of all types of studies to assess whether loss of vertical dimension has an impact on the appearance of temporomandibular disorders.Methods:The search strategies were performed in the Cochrane Library, Medline, Web of Science, Scopus, Lilacs, Scielo and Google Scholar databases using the following descriptors and / or words : "Temporomandibular Joint Disorders"; "Craniomandibular Disorders"; "Occlusion Vertical Dimension"; "Occlusion Vertical Dimensions"; "Vertical Dimension of Occlusion"; "Vertical Dimension". Results:A total of 4 articles were included in this review. Most of the studies show that in the presence of a reduced OVD, the stomatognathic system is able to adapt, not provoking the appearance of TMD. Conclusions:In conclusion, the results indicate that there is insufficient scientific evidence to show that the loss of vertical dimension predisposes to the appearance of signs and symptoms related to temporomandibular dysfunction (AU).

Introducción: Diversos estudios evidencian que las desarmonías del esqueleto provocan disfunciones temporomandibulares, pues la oclusión dentaria condiciona la posición de los cóndilos en la cavidad glenoidea. Los requisitos del tratamiento ortodóncico incluyen lograr una oclusión estable y equilibrada, una estética facial óptima y el funcionamiento saludable de la articulación temporomandibular. Objetivo: Describir los resultados obtenidos con los bloques gemelos en pacientes con síndrome de clase II, división 1, con trastornos temporomandibulares. Métodos: Se realizó un estudio descriptivo en los pacientes que ingresaron a la consulta de ortodoncia del policlínico "Juan Manuel Páez Inshausty", con edades entre 11 y 14 años, con síndrome de clase ΙΙ, división 1 y trastornos temporomandibulares, en el período comprendido entre febrero 2017 y enero 2018. Se describen 8 pacientes, con el análisis del índice de Helkimo al inicio y a los seis meses de instalados los bloques gemelos básicos. Resultados: Predominaron los pacientes leves con un 62,5 por ciento y el 87,5 por ciento presentó alteración en el funcionamiento de la articulación temporomandibular. A los 6 meses de tratamiento, se encontró que 7 estaban asintomáticos y solo un paciente persistía con alteraciones de la función de la articulación temporomandibular. Conclusiones: La mayoría de los pacientes pasaron a estar asintomáticos a los seis meses del tratamiento con los bloques gemelos(AU)


Introduction: Several studies show that disharmonies of the skeleton cause temporomandibular dysfunctions, since the dental occlusion determines the position of the condyles in the glenoid cavity. The requirements of orthodontic treatment includes achieving stable and balanced occlusion, optimal facial aesthetics and healthy functioning of the temporomandibular joint. Objective: To describe the results obtained with the twin blocks in patients with class II syndrome, division 1, with temporomandibular disorders. Methods: A descriptive study was carried out in the patients who entered the orthodontic clinic of the "Juan Manuel Páez Inshausty" polyclinic, aged between 11 and 14 year old, with class II syndrome, division 1 and temporomandibular disorders, in the period included between February 2017 and January 2018. 8 patients are described, with the analysis of the Helkimo index at the beginning and six months after the basic twin blocks were installed. Results: Mild patients predominated with 62.5 percent and 87.5 percent presented alteration in the temporomandibular joint function. After 6 months of treatment, 7 were found to be asymptomatic and only one patient persisted with alterations in temporomandibular joint function. Conclusions: Most patients became asymptomatic six months after treatment with the twin blocks

Introducción: Los pacientes con el Síndrome de clase II división 2, además de presentar clínicamente una relación de molares y caninos en disto-oclusión, muestran una retroinclinación de los incisivos centrales superiores con proinclinación de los laterales y sobremordida profunda, lo cual puede llegar a convertirse en un factor de riesgo que provoca disfunción temporomandibular. Objetivo: Identificar la relación entre el Síndrome de clase II división 2 y la disfunción temporomandibular. Material y Métodos: Se realizó una revisión bibliográfica automatizada en las bases de datos de los sistemas MEDLINE, PubMed, Hinari y Google; se utilizó un total de 17 artículos. Desarrollo: Por las características clínicas de la oclusión en los pacientes con este síndrome, el rango de movimiento mandibular está limitado, y produce un efecto traumático que se relaciona con una posición de los cóndilos desplazados hacia atrás e intruidos en la fosa glenoidea. Este desplazamiento más distal produce la presión del espacio retrodiscal ricamente inervado y sería causa de inflamación de los tejidos circundantes y afectación de la función de la articulación por elongación de los ligamentos discales o adelgazamiento del disco que afecta el complejo cóndilo-disco. Conclusiones: Los estudios consultados muestran la relación entre el Síndrome de clase II división 2 y la disfunción temporomandibular, dada las características clínicas presentes en este síndrome, que afectan el funcionamiento de la articulación temporomandibular y que al sobrepasar las capacidades adaptativas del paciente producen la disfunción en la misma(AU)


Introduction: Patients with Syndrome type II division 2 malocclusions, besides presenting a clinically established relationship between molars and canines in a distal occlusion, show a retro-inclination of the upper front teeth with pro-inclination of the lateral incisors and a deep overbite, which can result in a risk factor of temporomandibular dysfunction. Objective: To identify the relationship between Syndrome type II division 2 malocclusions and the temporomandibular dysfunction. Material and Methods: A bibliographic review was carried out through a search of databases such as MEDLINE, PubMed, Hinari, and Scholar Google. A total of 17 articles were used. Development: Because of the clinical characteristics of the occlusion in the patients with this syndrome, the range of jaw movement is limited, producing a traumatic effect that is related with a position of the condyles displaced backward and with glenoid fossa intrusion. This more distal displacement produces the pressure of the richly innervated retrodiscal tissue, which could be the cause of inflammation of the surrounding tissues and the affectation of the joint function due to an elongation of the discal ligaments or a thinning of the disc affecting the disc-condyle complex. Conclusions: The studies conducted demonstrate the relationship between the Syndrome type II division 2 and the temporomandibular joint dysfunction, given the clinical characteristics that are present in this syndrome, which affect the functioning of the temporomandibular joint, and produce its dysfunction when exceeding the adaptive capacities of the patient(AU)

Introdução: A disfunção temporomandibular apresenta etiologia multifatorial, enquadrando-se nesses fatores etiológicos condições psicológicas como a ansiedade. Objetivo: Identificar a prevalência da disfunção temporomandibular, ansiedade e seus respectivos graus nos graduandos. Métodos: Trata-se de um estudo do tipo transversal, observacional, com abordagem indutiva, adotando como estratégia de coleta de dados o Índice Anamnésico de Fonseca e o Inventário de Ansiedade Traço-Estado em uma amostra de 185 estudantes de Odontologia da Universidade Federal de Campina Grande, Patos, Brasil. Os dados foram trabalhados pela estatística descritiva e submetidos ao teste estatístico Qui-quadrado e Exato de Fisher considerado significativo ao nível de 5 porcento. Resultados: Os resultados indicam que a maioria da amostra é composta pelo sexo feminino (67 porcento) com idade média de 21,4 anos. Além disso, 79 porcento dos alunos apresentou algum grau da disfunção, tendo a maior parte (72 porcento) se encaixado no grau leve. A ansiedade estado e a ansiedade traço foram mais prevalentes no grau moderado com 79 porcento e 72 porcento, respectivamente. Não foi identificada associação estatística significativa entre a presença e ausência de Disfunção Temporomandibular e ansiedade, seja ela traço ou estado, na amostra estudada com p= 0,484 e p= 0,297, respectivamente. Conclusão: Conclui-se que houve uma alta prevalência de Disfunção Temporomandibular e ansiedade nos graduandos de Odontologia, destacando-se, assim, a importância da busca detalhada desses sinais e sintomas durante o exame clínico(AU)

Métodos: Se trata de un estudio del tipo transversal, observacional y con enfoque inductivo. Se adoptó como estrategia de recolección de datos el índice de anamnesis de Fonseca y el inventario de ansiedad rasgo-estado en una muestra de 185 estudiantes de Odontología de la Universidade Federal de Campina Grande. Los datos fueron trabajados por la estadística descriptiva y sometidos a la prueba estadística de chi cuadrado y exacta de Fisher, considerado significativo al nivel de 5 por ciento. Resultados: Los resultados indican que la mayoría de la muestra eran del sexo femenino (67 por ciento) con una edad promedio de 21,4 años. Además, 79 por ciento de los alumnos presentaron algún nivel de disfunción, y la mayor parte (72 por ciento) se encuadró en el nivel leve. La ansiedad estado y la ansiedad rasgo fueron más prevalentes en el nivel moderado con 79 por ciento y 72 por ciento, respectivamente. No se identificó ninguna asociación estadística significativa entre la presencia y ausencia de disfunción temporomandibular y la ansiedad, sea rasgo o estado, en la muestra investigada, con los respectivos valores de p= 0,484 y p= 0,297. Conclusiones: Hubo una alta prevalencia estadística de disfunción temporomandibular y ansiedad en los estudiantes de Odontología, así que se destaca la importancia de la búsqueda detallada de estos signos y síntomas en el examen clínico(AU)


Introduction: The etiology of temporomandibular dysfunction is multifactorial, and causal factors include psychological conditions such as anxiety. Objective: Identify the prevalence of temporomandibular dysfunction and anxiety as well as their levels among dental students from the Federal University of Campina Grande. Methods: An inductive cross-sectional observational study was conducted of a sample of 185 dental students from the Federal University of Campina Grande. Data collection was based on Fonseca's anamnesis index and the State-Trait Anxiety Inventory. Data were analyzed by descriptive statistics and subjected to the chi-square statistical test and Fisher's exact test, with a significance level of 5 percent. Results: Results show that most of the sample was female (67 percent) with a mean age of 21.4 years. On the other hand, 79 percent of the students had some degree of dysfunction, which was mild in most (72 percent). Anxiety state and anxiety trait were more prevalent on the moderate level with 79 percent and 72 percent respectively. No significant statistical association was identified in the study sample between the presence and absence of temporomandibular dysfunction and anxiety, be it trait or state, with values of p= 0.484 and p= 0.297 respectively. Conclusions: High statistical prevalence was found of temporomandibular dysfunction and anxiety among dental students, hence the importance of a detailed search for those signs and symptoms during clinical examination(AU)

ABSTRACT

Introduction: As a multifactorial disease, temporomandibular disorders (TMD) require a complex therapeutic approach, being noninvasive therapies the first option for most patients. The aim of this study was to perform a systematic review to analyze the most common non-invasive therapies used for TMD management. Methods: The review was done by searching electronic databases to identify controlled clinical trials related to pharmacologic and non-invasive treatments. Of all potential articles found, 35 were included in this review. Results: Low-level laser therapy (LLLT), occlusal splints (OS) and oral exercises/ behavior education (OE/BE) were the most common therapies used. LLLT showed significant results in pain and movement improvement in most studies. OS was usually combined to other therapies and resulted in improvement of pain. OE/BE showed significant results when combined with ultrasound, LLLT, and manual therapy. Conclusions: Non-invasive treatments can provide pain relief and should be prescribed before surgical procedures. LLLT was the therapy with the higher number of studies showing positive results. Based in heterogeneity of treatment protocols, diagnostic and outcomes criteria used, new well-designed randomized controlled trials (RCT) are necessary. (AU)

Introdução: A disfunção temporomandibular (DTM) consiste em um conjunto de condições musculoesqueléticas e neuromusculares que envolve o complexo articular temporomandibular, a musculatura e os componentes ósseos adjacentes. Sua etiologia inclui múltiplos fatores desencadeantes biológicos, ambientais, emocionais, sociais e cognitivos. Objetivo: O trabalho objetivou realizar uma revisão de literatura a respeito dos métodos de diagnóstico englobando uma visão biopsicossocial da DTM. Materiais e Métodos: Constituise em uma busca bibliográfica realizada em uma respeitada base de pesquisa online para a produção do conhecimento em saúde: PubMed ­ U.S. National Library of Medicin, limitando-se a busca ao período de 2005 a 2019. Resultados: Devido a diversidade etiológica e características clínicas variadas, a análise biopsicossocial individualizada associada à exames imaginológicos surge como uma relevante ferramenta para o diagnóstico desse distúrbio. Conclusão: Uma abordagem de diagnóstico com base em um modelo de doença biopsicossocial é fundamental para que seja possível avaliar o paciente em toda a sua integralidade física, psicológica e social com o propósito de efetivar o planejamento de intervenções preventivas e terapêuticas sob medida para cada caso clínico.


Introduction: Temporomandibular dysfunction (TMD) consists of a set of musculoskeletal and neuromuscular conditions that involve the temporomandibular joint complex, the musculature and the adjacent bone components. Its etiology includes multiple biological, environmental, emotional, social and cognitive triggers. Objective: This study aimed to perform a literature review on diagnostic methods encompassing a biopsychosocial view of TMD. Materials and Methods: This study is a bibliographic search conducted in a respected online research base to produce health knowledge: PubMed - US National Library of Medicine, limiting the search to the period from 2005 to 2019. Results: Due to etiological diversity and varied clinical characteristics, individualized biopsychosocial analysis associated with imaging exams is a relevant tool for the diagnosis of this disorder. Conclusion: A diagnostic approach based on a biopsychosocial disease model is essential to assess the patient in all their physical, psychological and social completeness with the purpose of planning preventive and therapeutic interventions tailored to each case. clinical.
